Part eight of the IEP is often neglected. This is your child’s transition services. IDEA requires that transition services be included in the IEP during the year that your child turns 16, Maryland lowered this age to 14. This is the transition for what happens after high school. Autism Speaks Family Services has a has a great document, Transition Information: Maryland, that includes a timeline of what should be occurring each year in school and out of school. It also includes information on Guardianship/Conservatorship, Vocational Rehabilitation, Social Security, and Housing resources.
Montgomery County Public Schools has a Transition Services Unit. The page has lots of resources for parents, including information about parent workshops, publications, frequently asked questions, and County, State and Federal contacts. There is even a Secondary Transition Planning Guide.
It has been my experience that most schools do not fully complete the transition section of the IEP. When I have seen it done, it is not complete or realistic. Transitioning from high school is hard for all students, especially ours with special needs. It is so important for parents, guardians and students to know what resources are available and to make sure the school is developing and implementing an appropriate transition plan.
